The dose can be titrated rapidly over a few days. It is not necessary to monitor plasma concentrations of the drug. Because it does not interact with other antiepileptic drugs, its addition to an antiepileptic regimen does not alter the plasma levels of other drugs significantly.
Safety and efficacy in children under the age of 12 years has not been demonstrated. No systematic studies have been done in geriatric patients, but dose adjustment may be necessary in elderly patients with renal insufficiency. See the Physicians Desk Reference for details.
Gabapentin has been shown to be fetotoxic in rodents, but this is not necessarily predictive of human toxicity. It should be used in pregnancy only if the benefits outweigh the potential risks.
Although these data support the safety of gabapentin use in pregnancy, the number of exposures so far is still small, and further studies are needed. Withdrawal symptoms from gabapentin, administered to mothers during pregnancy, have been reported in neonates.
As in adults, gabapentin should be gradually tapering off in neonates over weeks to months Transfer of gabapentin to breast milk is extensive, but plasma concentrations appear to be low in suckling infants and no adverse effects have been reported in neonates. Gabapentin does not interact with other antiepileptic drugs. It does not affect the efficacy of oral contraceptives used concomitantly.
Gabapentin is well tolerated and has a low incidence of adverse events in clinical trials. The most common side effects have included dizziness, fatigue, and headache. No life-threatening idiosyncratic reactions have been reported.
Dose-related neurotoxicity is usually mild. Gabapentin can cause dystonic reactions, which are reversible after drug withdrawal. In a postmarketing survey, neurologic-related events were the most frequently reported adverse events and were the most common reasons for discontinuing treatment.
Cases of gabapentin addiction have been reported, and most of these involved withdrawal symptoms. Childhood occipital epilepsy of Gastaut manifests with frequent and brief visual seizures consisting of elementary visual hallucinations, blindness, or both, commonly with postictal headache.
Motor seizures and hemiconvulsions can also occur. Onset is typically in late childhood or early teenage years. Valproic acid is used as an anticonvulsant drug to treat multiple seizure types, as well as for prevention of migraine and treatment of bipolar disorder. By , pregabalin had been listed as a new recreational psychoactive substance by EU agencies and in , Public Health England and NHS England wrote to prescribers calling for a balanced and rational use of these medicines among fears the pregabalin was being sold on the black market by those in receipt of GP prescriptions.
